Cost: $90 per person or $360 per foursome – Only Accepting Foursomes

the Farmington Country Club Golf Shop at the address above, Attn: JJ Amoruso; or, hand delivered. If your

team does not get in, no checks will be returned. All checks will be voided & destroyed.

CANCELLATIONS:

Cancellations received within 48 hours prior to the start of the tournament will result in the forfeiture of the full

entry fee. Cancellations must be completed online or by contacting the Professional staff at

434-245-0680.
